Does entering melee provoke an AoO?

Perhaps this is a silly question, but I can't find any rule that says entering into a threatened square provokes an attack of oppotunity. Does it?

If I do a regular move into a threatened square and then attack using my standard action, does the move provoke an AoO? What if I'm charging?

Leaving a threatened square provokes an AoO, not entering. Unless you have reach, you will not get an AoO on someone approaching you.
 

Entering a threatened square does not provoke an AoO. Exiting a threatend square provokes AoOs unless you are only taking a 5' step or using the withdraw action.

A medium sized creature attacking a large creature would provoke an AoO when entering a threatened square?
 

Creatures with natural reach threaten two squares. When a medium opponent leaves the more distant square to get to the closer one (the only square the medium critter can attack from), it provokes an attack of opportunity because it is leaving a threatened square.

Yeah, but NOT for entering the square, the large creature gets the AoO when the person LEAVES that first threatened square. A medium sized (or small) character with a reach weapon like a long spear gets an AoO against someone moving into a square adjacent to them as well.
