D&D 5E Fallout 4/Radiation Idea?

Any ideas on how to represent radiation and exhaustion mechanics from fallout 4 in D&D?

I'm guessing radiation is poison damage maybe necrotic. Or energy drain where it reduces your max hp.

Exhaustion would be exhaustion levels.

Think survival mode fallout 4.

I'd say radiation is radiant damage that inflict the poisoned condition on a failed save, while poisoned you max hp is reduced by 5. Multiple sources of rads can stack the HP loss. In case of doubt, look at the Sickening Radiance spell.

Exhaustion is indeed simply exhaustion as per the PHB.

Fallout 4 style radiation reduces your maximum HP, but notably it doesn't reduce your current HP if they are below that maximum.

I.e. if a character with 50 HP out of a maximum of 50 takes 5 points of radiation exposure, their maximum and current HP drop to 45, but if a character who is currently at 35 HP out of a maximum of 50 takes the same amount of radiation exposure, their maximum HP drops to 45 but their current HP remains at 35.

If you want to give it a damage type for resistance/immunity purposes, Radiant would feel oddly appropriate. :)
