Where are they located? I know of those in the Fly spell and in the feats section of the MM, but is there a consolidated set of rules somewhere?

Also, the Hover feat indicates that a character without perfect maneuverability must maintain forward movement each round (the feat negates this restriction). How much forward movement is required? If it's just a 5-foot step, then that's not significant at all.

The reference CCamfield gave answers your question. Minimum forward speed for fliers with average, poor or clumsy maneuverability is half normal speed. Those with perfect or good maneuverability have no minimum forward speed.
