The title says it all.
Here is a screen shot:
It does have a few house-rule items, like armor disadvantage for acrobatics as well as stealth and Death Toll, part for the most part it is pretty standard.
You should note:
I changed Animal Handling to Beast Handling, but it is the same skill.
There is no place for Alignment, height, weight, skin color, hair color, or eye color. We just put it under racial traits or other notes if you want it.
Printed size is 8" x 8", so landscape or portrait you are meant to have white space on the edges (or top/bottom) for additional information, tracking stuff, etc. I used it landscape showing two pages so I can see it as in the screen shot on my laptop.
