The non-dragon CR 20 monsters are the pit fiend, balor, and tarrasque, which have a +30, +33, and +57 attack bonus. That's not very hard to defend against. Your initial build means that the fiends need a natural 20 to hit you, and the tarrasque is not supposed to ever get an attack roll against you (it can't fly and doesn't have a ranged attack).

I'm not really seeing the problem.

If you think that there is a possibility that there is an invisible foe nearby, you can just attack an empty square. Attacking objects also works.

Another one nobody has mentioned yet: Mobility. +4 AC against AoOs due to movement.

I tried to convince my DM that my true neutral cleric should be able to get a magic item that provides both a sacred and profane bonus to AC, but he wouldn't go for it.
 


theres no need to attack empty spaces or objects to gain the benefits of fighting defensively. Simply take the 'total defense action' at the beginning of your turn (+4 AC, +6 if you have 5 ranks in tumble) and run toward the archers at your leisure
