I actually just adopted the d4+X system myself, with a minor difference:
d4 = d4
d6 = d4+2
d8 = d4+4
d10 = d4+6
d12 = d6+6
I wanted to make (most) everyone a little tougher, but also tweak the difference in average HPs across a couple particular classes. Doing it this way, fighters (which I see as slightly underpowered compared to clerics and barbarians) have a 2 point per HD lead on clerics on average, instead of the 1 they normally have, but still only average 1 less than barbarians. I felt like giving the d12 classes a 2 point advantage on the d10 classes was a little too much.
I do think this system hoses wizards and sorcerers a little bit, and I may yet change them to be d3+1 instead, but as my current party doesn't have one, I'm not worrying about it yet.
NPCs/monsters/etc still use regular old rolls.
