Basically instad of cutting the head off it just shattered it into so many pieces it was for all intents and purposes removed. We described it as head exploding, or doing a scanners.

And as a general rule one full attack at high levels from a fighter type, buffed cleric etc. is almost always a instant kill, it just requires a couple more rolls. I had a cleric that frequently did mid hundreads in damage every round he got a full attack off. Which basically meant unless we were fighting a dragon, or a really high con fighter type, it was a instant kill.
