Just last week our characters achieved 14th level in a campaign that has been running for 4 years and began at 1st level. That's the highest level I personally have ever reached in any campaign that fit your poll criteria.
 

2 campaigns 1st to 20th (well actually most of the players hit 21st at the end of the second campaign). Both took about year or so of play time.

I don't really have problem with high level games assuming there is a plot (which for me there always is).

My Freeport campaign started at 1st, has been going since 3.0 was released and the PCs are just hitting 17th level. We play every 4-6 weeks.

In the big campaign I ran starting when 3E first came out, one player went from 1st to 20th. Everyone else either joined later, died and remained a level behind, or gave up XP for some reason (item creation, house-ruled way of becoming a half-dragon, that kind of thing). Most were in the 18-19 range, though.
 

I played in one 1 to 22 game. It was an experience and I enjoyed it but the last six levels or so did start to wear on me. And I wasn't even a spellcaster! I think the revised haste would have helped a lot. There were lots of people doing lots of hasted actions all the time toward the end.

Of games I've run, I think 1-13 was the biggest span. I think I'm just about to start one that might go a similar length.
 

I have DM'd a campaign from the start of 3rd edition through all the WOTC pre-printed modules (Sunless Citadel through Bastion of Broken Souls and then some Epic adventures from Dungeon magazine too...) that has gone from 1st level to Epic. Currently the highest level character is 24th with a couple others at 23rd that have all started from 1st level as well.

I am also a player in another campaign that has run from 1st level to 20th level currently with four constant characters going the whole way.

It's been fun, but I also recently started playing another campaign to start some new PC's at 1st level running the Shackled City campaign.
 

The longest span of levels I've traversed (in a D&D or d20 game, mind; I'm not including other systems where advancement is in a different form or at a radically different rate) was, IIRC, 17 (1st to 17th), and the longest in a campaign I've GMed was 3rd to 21st. The former was over the course of two and a half years in a D&D/Iron Kingdoms campaign, in which my PC was the only original character to survive the whole campaign. The latter was over the course of a year in an Ivalice game running on a d20 Modern/OGL Conan hybrid.
 

The PCs in my campaign just earned 16th level. This is a new high for everyone. Ooo, boy. I'm dusting off the really big BBEGs... :]

2 PCs have made it to 19th level after starting at 1st. No small feat considering the number of dead behind them and tactics specifically aimed at eliminating specific characters or tactics. The leveling was gradual so building adventures was easier than starting at high levels, cold turkey so to speak.

Since the high level campaign was getting to be full of high-powered slug outs with the few remaining superpowers in the world, I brought in a new second campaign in the same setting, at 1st level. Those characters are now 2nd, and 2 have died.
