The Darkmantle was a total re-work of the 2E Piercer. I read that somewhere on the WotC website on an article they recently had.
 

I could have sworn that the Tojanda were from the Planescape monster book about the Inner Planes, but I don't have my copy here to confirm that.

I'm not sure, but most of these have very little 'traction' as new monsters.

I do like the chuul and grick, but none of the others have really come up, and some are just downright awful, both in a flavor and mechanics sense.

It was just called the Render back then, and it was unique. ;)

I like it better as a species, personally...

The spider eater is very similar to the 1e giant wasp (or "wasp, giant" if you're precision-minded).

There are more in later books, of course, but I think that list about covers the 3e Monster Manual (1).
 


If I'm not entirely wrong, the % of new monster would ROUGHLY be.

MM 8-10%
MM2 40-50%
FF 90-95%
MM3 100% (correction, probably 90-95%, forgot the 'Loths)
MM4 100% (not counting monsters who already appeared in MM in some form).

Those are rough numbers though...
