My favorite has been the Witchfire Trilogy. It had some small rules problems, and the campaign setting was introduced through modules, so it grew and adapted as the story did, but overall it has been the most enjoyable setting and adventure I have seen for the new editions.

The best adventure I have ever read, period is Tears for Twilight Hollow from Dungeon. Absolutely the best written adventure I have ever seen, mechanics and story perfectly matched.
 

I really like The Hunt: Rise of Evil campaign setting. I know it doesn't get much talk around here, but it's got some of the most creative ideas I've read in a campaign world in while.

Best adventure is a tough one. One of my favorites I haven't ran yet is The Magic Dump by Monkey God. Lots of wierd things in the swamp the PCs go through. It was a fun read, and had to have been a fun write for the author.

Monkey God made a lot of good ones. I'm partial towards "Maze of Screaming Silence". Despite the lousy name the module is really great (and nasty)!
 

I've really enjoyed running my group thru the Grey Citadel. It's got a good storyline going, and is flexible enough to appeal to different styles of play.

Before I created "Into the Land of Black Ice", an online 1e AD&D GH campaign started in 1995, and subsequently "Beneath the Pinnacles of Azor'alq", now a 3e D&D PbP which I began in 1998, I ran an offline adventure I entitled "Isle of the Unknown", where I took bits and pieces of B1 and made a L10-14 adventure out of them.
