Super basic idea, inspired by Zelda. Thoughts?

Heart Ruby
When interacted with over a short or long rest, this item increases your maximum health as though you gained a level. Roll your largest hit die+CON, and add that to your Maximum health. You do not gain an additional Hit die, and this item disintegrates after it is used in this way.

I've used a similar item in my games, except it was a necklace with a heart-shaped ruby, and it only granted the bonus for as long as you wore it.

I thought of that too, but wanted it to be more similar to the Heart containers from LoZ. Been working on a Zelda inspired dungeon for one of my players, and wanted a good item to put at the end. What better than the same item you get at the end of every other Legend of Zelda dungeon?

Yeah, but who's to say he isn't just loaded down with heart-amulets? The graphics never really go into, and the later games don't address the nature of a heart container at all. Just saying that it disintegrates after you pick it up seems kind of disappointing.

And I really like the idea of a physical object that you can pick up.

Yeah, they will be able to pick up, carry it around, and even sell or trade the item. It only disintegrates when it has been used over a rest, similar to attunement, but it will not take an attunement slot.

On Link, yep, it is totally possible he could be loaded down with amulets, considering how well he handles the other huge loads he has to carry. We will never know.
 

How long does the effect last for? Sounds permanent, which would be a pretty significant item, especially if it doesn't have to be carried or attuned or even carried once it is used.

How many can a single character use? It would seem to me to be akin to an artifact.

That was basically my plan. I was going to scatter a finite number across the world, to be found throughout the (hopefully) years we play in this world. I do plan for them to permanent, and one character could use as many as they can find. It may lead to an eventual character that has an extra 20 levels worth of life, but no character is permanent, so I am not so worried about that.

I think it fits well with your campaign concept (Zelda). You could consider some sort of ritual to "absorb" it or otherwise make sure that the characters know it's extra special.
