I think the text itself basically explains that you don't have to predesignate. It says the decision to use your reaction is "[w]hen a creature you see attacks a target other than you." It's at that point that you are designating who you protect (i.e. when an enemy attacks someone within 5' of you)."When a creature you can see attacks a target other than you that is within 5 feet of you, you can use your reaction to impose disadvantage on the attack roll. You must be wearing a shield.".
Unrelated note for Steve: you unfortunately can't reserve movement. Well, you can, but you have to use your action to Ready in order to do so.
