Can Graft Weapon be used on a Spiked Chain? It says any weapon, so I'd assume so.

So could I still use Claws of the Vampire on it? And if I Polymorph Self, what happens? I'd assume it'd still stay stuck to my hand.

Re-read the power description.

IMC I don't allow graft weapon with two handed weapons, since the idea just dosn't work for me. House rule, not at all in the power description, but I just can't see that working...

But if you allow it: Yes to claws of the Vampire, Yes to Expansion (remember: the only things expansion does is give you a enlargement bonus to Str and increase your height and weight. No size change, damage adjustment et cetera).

Polymorph absorbs equipment that the form cannot use. It also changes you, so No. Try grafting after polymorphing, instead.

But it doesn't do any different damage, nor extend your reach, or anything else. You do gain the Enlargement bonus to Strength, though.
