Once you roll initiative at the start of an encounter, the only way to change your position in the initiative tracker once combat has started is to hold your turn or ready an action.
The bonus to initiative is to an initiative roll, not the score. Say you roll an attack and miss, but a round later you get a bonus to attack. The attack that missed last round isn't retroactively changed to a hit because you got a bonus this round. The same thing happens with initiative.
Saagael: Yes, this. No-action powers that change you initiative right after you roll it will do what they say on the can, but otherwise bonus to initiative rolls only work if they're active when you make the roll.
