Regenerating Lost Bits

Inspired by the Beholder Eye Stalk thread, I wanted to post this question. How often does the "regrow lost bits" clause from Regenerate come up in your gaming group? Once, often, never? How did the incident happen? DM Fiat, Critical hit table, or something else?

Once, somebody lost a hand as punishment from a crime, and we were able to track down a scroll to help regenerate it.

We also had a character lose a thumb once, in a Pathfinder game. The DM was using random critical hit charts for that one. The character was a wizard, and the DM ruled a 10% spell failure chance on any spell with a somatic component. Given that we were level three at the time, and there was no way to recover that any time soon, we were fortunate that she (the wizard, not the DM) perished shortly there-after.
 

I use a "colorful critical hits" system that includes all kind of dismemberment, broken limbs, etc.

So it comes up a fair amount in my game.
 

In one high-level campaign, one of the players had a pirate-themed wizard with a peg-leg, while my character was a dwarven cleric of Pelor. The peg-leg thing was mostly just flavour and some skill-check penalties, and we tended to just view it as a character idiosyncracy.

At one point, about six months into the campaign, my cleric used Regenerate to heal up an NPC who'd lost an eye, at which point the wizard cornered him for a pointed and hilarious conversation that mostly ran along the lines of "You're telling me you could have done that at any time since we met?"
 

It has never come up in my game. In fact, I don't think I've ever seen regenerate cast. But our game simply does not have that sort of permanent injury - it simply does not happen.

(And, actually, one of the very few times I've simply told players, "no, you can't do that," came about when they wanted their characters to start chopping bits off a captured NPC. This was prior to "Game of Thrones" making such things routine.)
