D&D 5E The Confusulum in 5E

The Confusulum is an item from Simon Green's Once in a Blue Moon, but it seems to me to be a good fit for 5E, being an item from which everyone in the party benefits.

The Confusulum protects the identities of the wearer and their friends. It is an intelligent item, described as a locus for an extra-dimensional entity.

In 5E terms it functions as an Amulet of Proof against Detection and Location with the additional function of minor physical changes such as skin and hair colour, hair style, voice, facial shape, minor size change - a more limited form of Alter Self. . The Confusulum will keep the changes relevant long term as those attuned grow older. The changes can only be detected through Truesight. On top of this, the Confusulum radiates (120' radius) a mild Confusion / Forget effect to disguise mannerisms and the like; only a Mind Blank is proof against this. The Confusulum need not be always worn but must always be close. The protection provided by the Confusulum fades after one day away from proximity (30') to the Confusulum.

Once per day the Confusulum may be used to make a target forget the actual identities of those it protects. The save DC is 20 and the Confusulum imposes Disadvantage on the save.

Mindless creatures are not affected by the Confusulum's Confusion / Forget effect. In the book, this includes a headless axe-man.

To use the Confusulum you must persuade it to work for you. Or it may decide to work for you anyway. Once persuaded, you and others may attune to it.

As the locus of an extra-dimensional entity, that entity may use other powers through the Confusulum.

The personality of the device in the book seems fairly friendly, though it gets few lines. In Greyhawk terms, it would more likely be a device of Dalt or Zagyg rather than Syrul.
