Total Party Kill (Possible RToEE Spoilers)

Is there something about the Outer Fane that prevents parties from fleeing via teleport and similar spells? If not, why do so many people die there?

The party in my group doesn't go anywhere without at least two teleport capable party members (the cleric and either the sorcerer or the wizard). The wizard just picked up mass teleport for emergency evacuations, and many party members have items of recall (can't remember what the spell is, but it teleports the item's breker to either a specific location or the caster of the spell's presence.

I can understand if some party members die and the corpses are unrecoverable, its happened plenty of times in my game. What I can't understand is the TPKs. Killing an entire high level party is about as difficult as kill a single party member is easy.
