A Quickened True Strike would be a 5th level spell. A wand of it isn't too hard to imagine. Of course that would require it to be on your class list (or for you to have UMD.)

To actually make an item that does it ...

That would likely be expensive.

PS: How did you get your ? to go upside down?
 

No. I would not allow such an item into my games because it would make fighting classes lose their major advantage, their likelyhood to hit. What's the point of being a fighter if you can be a rogue with +15 better to hit AND sneak attack?

Or at least, it diminishes their specialty (I guess it doesn't eliminate it).

For that mechanical reason I wouldn't allow it.
 

Sure, I'd allow it. I'd price it at 25,000 * 20* 20 gp: a cool 10 million. (That is, he same price as a luck bonus that transcends the normal limit.)
 

Yes, and I'd use the rules as presented in the DMG to price the item.

I would then give the item to every NPC or monster that was capable of affording or using the item. After all, if the item is that cheap and that powerful, everybody would have it. Everybody.

Then I'd stock up on body bags that were custom fit for the PC's.

This would teach them that not every rule, item, spell, or PrC on a webpage or printed in a book is well thought out or balanced, or it would teach them to 'munch' even harder to survive.

Gee, the Hill Giant only needed a 7 to hit you before - now he can afford to Power Attack for 12 and still hit you on a 3 - while fighting defensively.

I don't think that is possible with Craft Wand. Wands can only have spells of 4th level or less...

Sure. No worse than any other magic item, no matter how much someone pisses and moans about it being "unbalancing". ;)
 

I'd make it use activated... quickened is way too limited! C'mon, that's only once per round! ;)

However, I'd allow an item, which can cast True Strike at will as a standard action, because that is pretty fair, really!
