Okay, what happens if your are truly flanking the PC from the sides or rears (rear left and rear right)? Do If Rogues X and Y are on opposite sides of a PC: one in front the other in the rear, what (if any) are the new flanking rules?

You still threaten all of the squares. However, if you attack either of the flanking squares, you are at a -5 to attack, and if you attack rear squares, you are at a -10. If you have the Combat Reflexes feat, the flank penalty disappears. If you have Whirlwind Attack, there is no penalty to any square.

There is a lot more to it than that, but that should give you an idea.

Is there a bonus for attacking someone from behind? Otherwise, these rules seem incredibly incomplete, to the point that I don't see a reason to use them.

Yes, there is. You no longer need an ally on the other side of the enemy to flank either. If you are attacking from a flanking space, you get a +2 to hit. If you are attacking from a rear space, I believe it's a +4.

As far as sneak attacks go, the facing rules variations state that you no longer get sneak attack bonus damage from flanking. But you can always get it from a rear space.

Oh, and from any space when the target is denied his Dex bonus to AC...

And the -5 and -10 penalties apply to spot checks too...(my favorite aspect of rules...)

Sounds like well thought out rules for the folks who use miniatures, but an unattractive variant for the more abstract combat players.

Though rogues sneaking up from the rear is a blessing. Yay.

The only question in my mind is this:

How easy is it to run around behind someone and hit them from there?

That's what tumbling is for...

I most definitely agree there. I think it would only confuse the game without minis.

So you're saying that a rogue should need only to make a tumble check every round to get their sneak attack damage?

Try this as an experiment. Tape a piece of paper to your buddy's back. Stand in front of him. Tell him to keep you from grabbing it. Try to grab it.
