I've replaced the 10% auto-stablize with a DC 18 Con check. So the average person has a 10% chance of doing so, but hardier folk have a better chance and weaker folk have a worse one.

I also require 10 rounds to accomplish first aid (5 rounds with someone aiding) and the chance of death occurs a -10 - Base Fort Save - Con mod. At that point you make a FORT save each round DC 15 +1/check or die. If you are brought below you "death number" in one fell swoop the difference is added to the DC of the save.

So if you die at -20 and get dropped to -25 in one blow, then it would be FORT save DC 20 +1/check.
 

The one major drawback I see with this is it makes con even more of an important stat as far as not dying, as now it effects both your hp and your fort to prevent death. And I already think con is the best stat in the game overall.

I like the fort save to stay conscious, but I personally would leave it at -10 you die. That way while beefy fighters can stay up they don't die at crazy numbers and people with poor fort saves don't die at -1 (you can always roll a 1, no matter who you are:)

Actually, rolling a 1 while at -1 won't kill you, as you still haven't failed the save by 10 or more, unless you have an impossibly low Con score:

Of course, you could always have other modifiers to the die roll, (an enemy prayer spell, for instance.

Exactly. You've failed, but you haven't failed by 10 or more. At worst, you fall unconscious, which you would have done anyway under the standard rule.

Take note that I purposefully designed the Fort save so that someone with a Fort save of +0 cannot possibly die until they get to -11 hit points or worse.

Also, in Iron Heroes, which is the game I'll be using it with, you do not automatically fail a saving throw on a natural 1.

I guess I also misunderstood that detail. That makes me like this method a lot more, though I'm still becoming a raving fanboy of el-remmen's rules.
