D&D 5E Werewolf Wizard

You can either have him play a Shifter (Eberton race with lycan ancestry) and just say his non-shifted form is Human... or do a feature exchange where you give him wildshape at level 2 instead of Arcane Recovery. Either method should work out.

I'd just do it as a Rage Feat ie the PC transforms into a Savage Wolf-man Hybrid while raging.

While in Wolf man form
You have advantage on Strength checks, gain a + bonus to damage and do claw damage, have Resistance to bludgeoning, piercing, and slashing damage.and can't cast spells while raging.
 

I’m just going to make it where he can transform whenever he wants but can’t cast spells in hybrid form or wolf form.
 

I'd give have the player two character sheet: one for the human form and one for the wolf form. The character automatically shifts into wolf form when under the light of the full moon and switches back when exposed to sunlight the next morning.

* Forms can have different abilities scores, classes, etc. The wolf form will have high physical stats and will probably take levels in the barbarian or fighter classes.

* Any damage taken in one form is carried over whenever the character transforms. Which means that if the character takes too much damage in wolf form and is switched back to human form, the character may die from his/her wounds.
 

Hmm. Yeah, there's the feel of power gaming around the edges there. The bigger the benefits he wants, the stronger the downside has to be. The ability to shapeshift is a core Druid ability and not only is it paired with a lesser spell list I'd guess the werewolf form is supposed to be pretty butt kicking. It will need very careful balancing. Maybe something like a Shifter from Eberron would be a more balanced way to approach that?

Agreed. Tweak the shifter race to something mutually agreed upon. Done.
