What do you do if a cohort (or even a high-level follower) has leadership? Do you even allow it? The PC could quickly find themselves commanding hundreds of followers and dozens of cohorts. Would you allow it? Also, if the PC was a noble or some other commander of people, then would you allow it, because they are supposed to command many people?

Leadership... being a wholly optional feat... is completely dependent on DM fiat.

But in your specific case... if you are DMing a military campaign, then yes.

Keep in mind that you can have followers and allies - even trusted lieutenants - apart from those granted by the Leadership feat.

The Leadership feat represents those people who not only follow you, but are personally devoted to you. They're your men [and women], come what may (until you kill them off, anyway).

Thus, a warlord might command an army of thousands, but his Leadership feat grants him one cohort and 50 NPC followers - and these likely represent the troops that started with him, form his Elite Guard, and have sworn to die when he dies.

It's possible, and I believe even noted somewhere as a good way to create a multi-tiered organization, such as a large Thieves' Guild.

As for what you do if it happens? You have a nice sit down with your DM, explain what it is you're aiming for with your nested Leadership pyramid scheme, and then you give him a big ol' bottle of painkillers for all the headaches that asking him to manage a gigantic menagerie of NPCs are going to cause him to suffer.

You can do it, but it also carries along that much more bookkeeping that has to be done.

What "I" do depends on what the player does. The NPCs from leadership are loyal to the HOLDER of the feat and behave as a living, sentient person. If the party fails to respect a player's cohorts/ followers, the cohorts/follower's underlings WILL take action to benefit thier masters.
