I'd charge 1,500gp. It follows the cost calculation formula and it's not too powerful of an item, so the price doesn't need to be adjusted. (bonus squared x 20gp)

+5 to Move Silently = 500gp (x2 because it's a similar power on a sloted item)

I'd charge somewhere between 15,000gp and 22,500gp. I am assuming the bonus to hit and damage is a luck or competence bonus, which are some of the most expensive bonuses you can have (bonus squared x 2,500gp).

+1 to damage = 2,500gp (x2 because it's a similar power on a sloted item)

As a side note, I would like to point out, however, command word items and use-activated items do not provoke attacks of opportunity when activated (unless the item description specifically states so).
