No problem, I was the one throwing the die and I still don't believe it happened. I was using a dice cup at the time so I had no real way to manipulate the roll. I also switched out the die three times during the chain of throws (two of which were not mine so there was no way I could have rigged them beforehand).

Odds for rolling dice in a row are a fallacy because no throw before or after can affect another throw. That is why we use dice in the first place, you can not normally predict the roll before it is made unless the dice are rigged/damaged (though how a die is thrown can affect how it lands).
 

For a character-
When Psionics 3.0 came out I promised myself to use the first six rolls as is to create a human psychic warrior. The DM didn't believe me and disallowed the character which to this day annoys me since they were TRUE rolls- no pretending or redos. However, to be fair- I was by myself and the rolls were-

18, 18, 18, 16, 14, 12

I can only think of three charcters in my 15 years of playing I have ever rolled an 18 for a stat. It truely sucks.

As for damage-
Psionic Stone Giant with charged up rock hit for near max damage a character too close to a cliff. I had him roll a d20. 1-4 you fell. Rolled a natural 1. "Quick! d20 dex to grab something another natural 1. Between the boulder (psionically charged), the fall and at this point, the impact of the other giant he was fighting that also fell the damage was over 150.

Too thsi day we still laugh about it. He was -100 hp in the end (er bottom).
 

My players didn't like the RP only session we were having, and demanded an encounter. So I pitted the 8 or so 3-5th level PCs against some stock MM orcs. First round, tossed some javelins for minimal damage. The wizard taunted the orcs, who pulled out great axes and charged.

1st orc, crit, 44 points of damage (out of 45 max), 5th level wizard goes to -10. 2nd orc, crit, 42 points of damage, 3rd level fighter goes to -10.

2 more crits during the fight. Nearly a TPK, and when they finally win, 75 XP each. :]

well I dont remember the exact rolls but I had a ordinary bugbear survive an encounter with 8th level characters. He was supposed to be cannon fodder but he outgrappled the monk twice! then made two saves, one vs a hold person and one vs a fireball that rolled poorly and left him with 3 hp. He was running around shouting
"Im Mr Lucky! no one can kill me, cause I am Mr Lucky!"
He was finally taken down, but of course he made his stablization roll and the Pcs were chased off before they could loot the bodies and finish him off.

Not exceptional rolling but my 14th cleric/1st lvl ftr had just finished destroying a temple in CoSQ - and a balor was sent after him in retaliation. The rest of the party distracted it long enough for me to get a few buffs up. DP, RM (nerfed), and a quickened DF. I hit then critted the Demon with a dire pickaxe (d8 x4crit) 138 dmg, with my last attack hitting only AC 28 missing. (it was in my sig for a while)
the demon teleports away to beat down another PC. I had magic boots and charged - another crit for 101 damage. Boom. The DM ruled there was part of my body remaining at -39 hp.
 

Axis and Allies

Ok, not Dnd (Axis and Allies, you know, the board game). We drew cards as to who was going to play who and 2 brothers (the best at the game) drew Germany and Japan. The other 3 of us looked at each other and figured we were toast. I drew Russia (my bad luck). Germany attacked into the NE Russian territory with just about everything it sould (forgot the name of the territory, but it has the industrial complex and the like). He rolled and with everything, infantry (I think 8-10 or so), tanks (another 8 or so) and planes (2) killed like 2 of my infantry (I think I had 12 infantry and 2 tanks). I rolled back and had something like 6 or 7 kills. Painful to him, but he kept on going. Another round for him netted a grand total of 1 or 2 kills... I finished off with rolling 75% hits, wiping out all of his infantry and nearly all of his tanks - and lost a couple of infantry in total... Needless to say, Russia took over most of Europe that game...

If you truly believe that, I hope you don't use percentile dice ... rolling two 10-siders to generate a number from 1 to 100 is entirely based on "odds for rolling dice in a row."

It is, of course, true that any given die throw is an independent event. However, it's likewise true that any given group of dice throws -- when considered beforehand -- is also an independent event. The calculation of probability for the outcomes of both is therefore entirely possible and proper.
