How about letting the PC use the Regenerate spell (self only) as a spell-like ability 1/day?

Or maybe something along the lines of the Monk's Wholeness of Body class feature, usable as a swift or move action.

Or simply give the PC Regeneration 5, but limit the number of rounds/day he can have the effect up (how about as many rounds as his character level, doesn't have to be continuous?).

Those are still nice abilities to have, and warrant the use of a powerful effect like Wish, but are far from gamebreaking.

Actually... the problem here is that you're allowing the player to wish for a "mechanic" that the character would not be aware of. The best they could say is "I want to heal very fast!" or something along those lines...

the regen could also come with a disadvantage.

congratulations! you now have regeneration 5!
your constitution score is now 5 points lower.

I agree with Empath Negative, fast healing 5 was a metagame request.
How did the character word the wish, half the fun is the chance to twist the words if the player asks for too much.
"i want to regenerate like a troll" should have the DM rubbing his hands with glee

The "twist the words" thing is a holdover from earlier editions, also known as D&":):):):)-the-players-over-as-hard-as-you-can"&D. I'm resolutely opposed to using Wish spells that way. It's an extremely costly spell from the highest spell level, it should have positive effects, period.

Metagame request? Yeah, sure. The metagame is half the game, and in a spell as open-ended as Wish, metagaming is just part and parcel of the player-DM negotiations that must take place with nonstandard wishes. That's just how it works. You CAN do it differently, but IF you do, please don't "rub your hands in glee" if the player doesn't get his lawyer to approve his wish wording first. That's just demeaning, plays up a DM/player adversity that shouldn't be there, and leaves everybody except the gleeful DM having less fun than they could have.

Here is what I suggest: let him wish for "regeneration" and then you, as DM, can set the parameters based on what you think is fair given the power of the spell. Be it regen 0.5, regen 5 or regen 50.

The twisting the wish thing comes from bottled genies who screw you over. Thematically it shouldn't be used for someone casting a wish spell themselves.
