I think it will be a new setting, maybe on of the two finalist of the setting search...
I'm excluding dragonlance or any other vanilla fantasy setting, because this genre is already covered with the Point of Light implied setting and with the Forgotten Realms.
Planescape will have already three or four books (planar manual, planes above, planes below etc...) so I don't think they will make a planar or spelljamer setting : already covered.
It could have been ravenloft, but it's already developed in Dragons and will be probably covered with the shadowfell sourcebook.
kenmarable's idea seems great, but I can't just say "like he said", so I will try to imagine something else...
So, what could we have ? Light fantasy à la Jack Vance's Lyonnesse ? No.
But... What about an horror setting ? There is a lot of stuff about the Far Realms and its creeping invasion of reality in 4e. Mix that with the D&D super-heroic tradition, and you may have a setting where the PC belongs to various secret orders dedicated to defend/free the world from the abomination. It could be somewhat "WoDish", with the PC choosing factions (the various secret orders) that can bring them mecanical advantages and involve them in complex intrigues.
