D&D 5E what are your top three "$#%#@$ stop forgetting this rule" annoyances?

In the game I’m currently DMing, one of my players is playing a Paladin and absolutely cannot seem to grok Lay On Hands. He understands that it’s a resource he can spend to heal himself or a character he’s touching, but every single time he wants to use it, he has to ask how, it usually takes explaining it multiple times, and even then he looks confused. I have no idea where the disconnect is, but something about that ability clearly just doesn’t make sense to him.

Here are my pet peeves (things that players do):

1. Not knowing your character's class abilities, and then asking me what you're capable of. That's not my responsibility! My responsibility is to create cool stories and encounters in interesting locations, not to act as a rules encyclopedia. This is exacerbated by the fact that I have an excellent memory, and actually can act as a rules encyclopedia in most circumstances.

2. Ditto on hit dice.

3. Conveniently 'forgetting' to make checks to maintain concentration.

4. Players who are unable to manage one class resource, but then get upset when I, before quickly correcting myself, accidentally skip one turn in combat. Do they not know how many things I'm trying to do at once?

I'm not sure what's worse.

Players that don't know what their abilities do. OR ...

Players that forget what their abilities are.

I have a player that regularly plays fighters, and always (ALWAYS) forgets that he has action surge. I can't even imagine what would happen if he played a spellcaster!

I didn't realize this was even a thing until I hit it as well. I have a player who's PC is 5th level, I had to remind him to make two attacks per turn. I've given up on reminding him he has action surge, maneuvers (I only know he's a BattleMaster because I shared my DndBeyond account) and can use a healing surge as a bonus action.

He's always "saving" things for later.

Yeah, concentration on a lot of spells seems to slip through the cracks. It is one of the most annoying things about 5E IMO and I prefer spell duration from earlier editions for every spell.

TBH, my folks would forget durations also.

Oof. I know. Sometimes, when people complain that the poor ol' Champion, and even the Battlemaster or various types of Barbarian are just way too simple for any regular player ...

I have to wonder if they've gotten out much. Because, you know, people.
The main problem, the origin of all of this strife, is probably the fact that DMs are the most invested in the game, generally know the rules well, and spend more time than most players involved in the game.

The involvement of the players, well, most of them, in the game, seems trite by comparison.
