D&D 5E Hex Shenanigans

Context matters. Saying a character could never survive a direct grenade blast IN A SUPERS GAME is ridiculous.
The chunky salsa rule would obviously scale differently in a supers game. I think it might be more like the Flamin' Hot Habanero and Lime Chunky Salsa rule, and apply to things more like direct nuclear strikes and falling asteroids the size of Delaware.

Is Collosus jumping on a regular grenade going to auto kill him?

Would it kill wolverine?

Even a "normal" human character like hawk eye or black widow, if they jumped on a grenade - we would assume they new some trick or gimmick that would make the blast less than fatal. The genre provides that these kinds of things are ok.

What about the aforementioned meteor strike/ nuke?

Or are superheroes just by default completely invulnerable with no weaknesses?

Will a Meteor strike or nuke do enough damage to kill the character? If yes then it kills them. If they have enough HP/resistance (whatever the game mechanic) to possibly survive it, then maybe they do.

It's not that complicated.

Edit: ESPECIALLY in a genre where such things are known to happen - and are not even that uncommon.

Character creation is a much a player's decision as it is a DM's decision. It must be a cooperative thing. Otherwise you might end up with things like: " I am the Wizardking's heir, I start with a staff of the archmagi, a robe of the archmagi and an amulet of health." kind of sillyness. Or having evil PCs alongside good aligned pc. This is a receipe for disaster and PCs infighting. I would never encourage such a behavior at my table. Characters are build with the DM and the players. They all decide which characters will be made and who will play them. At session "0" we talk about the kind of group they want, the orientation and the campaign type that will be played (only the general theme).

What are you talking about?

The parameters include character level and magic items. The player creates their PC within those parameters.

If the character level for this campaign is, say, level 5 and the players can choose 1 uncommon magic item (subject to DM approval), then the Staff of the Archmagi is not within those parameters. Nor is 'I'm the finest swordsman in the world' likely to be possible at that level.

Man, if a player told me his fighter was going to jump off a cliff I'd give him a warning that this would kill him. I don't care if the rules say the fall will only do 15d6 and I can't possibly roll high enough to kill the character. I will flat out kill the character should he choose to jump. But, like I said, I'd warn the player first.
In many, many action films the heroes are being pursued by an overwhelming number of foes when they come to a cliff edge. Indiana Jones (or whoever) looks back at the chasing horde, looks at the leading lady, takes her hand and takes the seemingly suicidal leap.

Of course, Indy is experienced and lucky and the Big Damn Hero. Of course they survive. They have the hit points, obviously. : )

Player: Indy looks back at the horde, looks at the Girl of the Week, takes her hand and jumps. Indy has 120 hit points and the girl has 90. We should survive unless we are very unlucky, but we will be in bad shape....
MGibster: No. You're both dead. I don't care what the rules say, I don't care that you as a player made a calculated risk or that Indy in-game made a calculated risk of fall versus horde. I rule that you are dead and I'm the DM so there's nothing you can do.
Player:.....Thanks for the fun game....?
