My DM decided that he had severe headaches and started to feel bad and died.

The others became suspicious

Yeah, people are often suspicious when their colleagues drop dead for no reason. I totally expect my colleague to search the office for 24 seconds then resume typing reports on his computer should I die after complaining about a crippling headache a few seconds earlier.

My DM essentially forbids any attempt at Metagame. If your player knows, it doesn't mean that your character knows. That simple.
You lose XP certainly.

I don't think 5e has provisions for losing XP. Are you sure you're playing the same game as the rest of this board?

You do not know the location of the attacker, the spell is imperceptible and you only know that you took damage.
Simple. Roll initiative is useless here.

You know somebody cast a spell on you, because you tried to mentally resist it (i.e. made a saving throw) and took damage. You also know that whoever is attacking you is unseen, i.e., probably outside your 60' darkvision. Very first thing you do is put up Dancing Lights, which you can move 60' as a bonus action. So you can sweep out 80'x60' zones of darkness every round. And there are four of you.

What I do as a DM to avoid metagaming myself is roll to see which direction the monsters sweep out first. Assuming they're not in an infinite plane, it shouldn't take long. And, of course, the first time you cast a spell without Subtle Spell, you're dead.
