Planescape is Jeremy Crawford's favourite D&D setting. "It is D&D", he says, as he talks about how in the 2024 core rulebook updates Planescape will be more up front and center as "the setting of settings".
Known Permanent portals tend to be guarded, and access only granted for a select few. Lots of people don’t know wizards.They say that, but it wasn't at all true. The 2e Planescape boxed setting included the 2nd level wizard spell Warp Sense which not only found portals for you(and there are thousands in Sigil), but also allowed you to figure out where the portal goes and what the key is to make the portal function.
Then there are all the known permanent portals about the city.
It's super easy to get out of if you have a wizard or talk to the right folks.
As hard as finding a level 3 wizard with the Warp Sense spell, which would be very common given the level of the spell and its importance to Sigil. I just don't think that I'm doing something wrong in my play of D&D if I don't imagine my world as cosmologically connected to all these other worlds.
During the pandemic, I actually came across the Revised-era book they put out on the Inconnu - well worth a read I think, as it might be the first time I read a Vampire book that felt like something I could actually run at a table for players. Seriously, it was almost surreal to read something for that system that I could use to play a game.Yeah this. We ran some official stuff for Vampire for example, but none of it really furthered or even relied upon the metaplot. The Time of Thin Blood came closest, but who even wanted to play a thin-blood? Not any player I ever met.

I fail to see where the problem of "it's all D&D" and "mix and match what you want" is. If you want a curated list, be my guest. WotC is serving a huge buffet and you are free to take what you want. Even if you want mashed potatoes and gravy on your waffle.

I think "lazy" and "DM" are words that can never correctly appear in assotiation with each other. But nothing here makes that sort of thing any more likely. The D&D multiverse is where a character travel from the Forgotten Realms to the World of Greyhawk, or from one DM's FR game to a different DM's FR game.
