Pineapple Express: Someone Is Wrong on the Internet?

I like this coworker, but it's frustrating because it doesn't seem like they're fully aware of the issue, beyond a jokey "ha ha, I'm such a flake" kind of way, despite having a role with a lot of responsibility.
A lot of ADHDers cover with humor. Getting cranky at him won’t help. If it is ADHD he likely beats himself up about it more than you ever could.
I would like to help them find some strategies that would work for them, but that feels like crossing a whole lot of lines.
I’d suggest starting where you can. Adjust your expectations. Give him more slack and follow conversations up with a written recap via email. Don’t rely on his memory when you know it won’t work.

They don't seem like the self-medicating type -- although obviously, you never can tell -- just shockingly unable to remember what was discussed 15 minutes before, including contradicting their own ideas.
You will think I have a drug problem, then, if you ever met me. I have real issues with long-term memory. I will get up early to do something, remember it perfectly upon waking up, and then lose track sometime during my morning shower and end up leisurely making coffee in the kitchen before somebody calls, asking "Where the heck are you?" I will forget my I went into a room, even if the room I was in is on fire and there's a fire extinguisher on the wall in the room I just went into. I can barely remember to take my life-preserving medication I need to take EVERY DAY.

I suspect it has to do with diabetes, and that the sugars I lack sometimes would have been used to form long-term memory bonds in my brain.

Sometimes it is good to temporarily unblock someone just so you can remember why you did it to begin with.

Sometimes it is good to temporarily unblock someone because they turn out to either be not as bad you remembered, or maybe they matured some in the intervening time (or you have).

LOL.

And sometimes you unblock someone and discover that they are still stalking you several years later.

Some people really, really have a tough time letting things go.
