My DM does this about once every two sessions, it usually works and doesn't actually disrupt as much as a DM scrambling directly in front of the players for just about anything.
When I master, I've never done this. I either say something like "whawhawhat?" and then use the first thing that comes to my mind. This takes about 1 second and works most of the time. Once I've broken a long Star Wars Session into chapters and made a break after every chapter to gather my thoughts.
 

I tend to go with a pre-determined "random" encounter :P

Surprise me, damn yous... I'll throw trolls at you! (or various types of ability draining Undead...)
 

I didn't think about my own DMing habits when I posted before.

This is where I have a big advantage. I DM online, using Fantasy Grounds, so the players can not actually see me panic and flip thru books. The can think I am calm and prepared for everything they throw at me.
 

I have a big binder full of extra encounters, quick and easy plot hooks, random events (you wouldn't believe how upset a monk player got when I said he stepped in a gopher hole and twisted his ankle - no damage just a slight limp for a couple of hours). If I get surprised by the PCs actions (which doesn't happen often) I whip out a few of those to give me some breathing space and time to develop whatever I need to keep the game fun.
 


I try to anticipate what why players will do but sometimes they come up with something totally from left field. Like last night. The PCs were invited to a party at a well to do playwrite's house. They know that the BBEG is either the playwrite or is working for him. One of the PCs (a gladiator) is hired as a security guard for the party. His job is to patrol the grounds and keep pesky people out. During the evening, part of the group decides to go outside to find the storage shed they believe is holding someone captive. Once outside, everything goes to heck. the gladiator PC hears someone moving and decides to investigate. It is dark out with very little light. The gladiator PC had earlier in the day (unknown to the rest of the PCs) cleaned up and shaved and aquired a new weapon of choice. After several move silently and hide checks, the galdiator and one of the PCs encounter each other. One of the PCs fails his spot check and doesn't recognize his gladiator friend. He sneak attacks him. The gladiator, having just been attacked, turns and retaliates. After more failed spot checks, the fight continues until one of them is at neg 6 HP. The gladiator then hides the body. The other PC, having heard the battle from his hiding place, takes off.

Ah, the joys of option Q.
