Character Campaign Books

Wondering if someone here can help me. In my old age I tend to forget things. I recall a few years back seeing a character campaign booklet that was about 5x8 or similar dimensions that had pages for the player to fill out that detailed nearly everything about the character in one handy little booklet.

Now I use player binders right now and they work ok but tend to take up a lot of space as well as the multitude of laptops and iPads littering the table. I was wondering if anyone knew what those booklets were called as I am in need of finding them to use.

First of all, welcome to EN World!
To your question, a few different products such as you describe have been put out over the years. The most recent one in my experience was the Character codex put out by Goodman Games.

I thought the handle looked familiar but I'm on so many gaming sites that noting a recognizable handle but being unable to place it isn't all that unusual.
FWIW, I believe paizo has put out or will soon put out a similar type product if that's of any use to you but I don't have a link handy.
