I may be misunderstanding your question, but as far as I know there is no rule anywhere in the core books that says a GM
must allow a character to create a custom item. And since no item in the DMG (other than artifacts, I suppose) qualifies as epic, this shouldn't be a problem. In fact, reading the RAW, only the
GM can create a custom item. DMG, page 214, sidebar:
VARIANT: NEW MAGIC ITEMS
In the same way that you can invent new spells and monsters for your campaign, you can invent new magic items.
Note the reference to "monsters" and "your campaign", clearly implying that the passage refers to the GM, not the players. If a player wants his/her character to be able to create a new item, then the GM has final approval, per the RAW. In effect, each non-core item becomes a campaign-specific house rule that the GM must approve.
So if a player wants to create a Bow of True Striking, the GM is well within the rules to say, "Sorry, that item doesn't exist in the rules and I'm not willing to add it to my campaign".
