Yay, well, since Shade beat me to the Yyllethyn, Allow me to attempt the Crayghe, or Elven Hawk.

This pwerful bird looks like a hawk, but it is larger, and has a spark of intelligence in its eyes. The bird lets out a piercing cry and circles overhead.

The Crayghe are a rare breed of hawk trained and domesticated by elves. Few Crayghe live in the wild. The crayghe are intelligent and able to understand simple commands in elvish.
Crayghe do not stray from their elven masters, and they are able to survive almost anywhere there is game for them.

Combat
Crayghe begin combat by diving onto their foes, and then they go after the target's eyes, attempting to blind them.
Blinding Attack: When a Crayghe attacks with its beak, it goes for the eyes of its foes. If a crayghe deals damage with a bite attack, the target must make a DC 10 Reflex save or be blinded in one eye due to eye damage (See damaging specific areas on Page 27 of the DMG). If the save is failed by 5 or more, the blindness in that eye is permanent, otherwise, the blindness lasts until the target has regained at least 1 hit point. Permanent blindness can be healed by a heal or regenerate spell. The save DC is Constitution-based.
Diving Attack: A crayghe that charges a foe from a height of 30 feet or higher, deals double damage with its talons, in addition to the normal benefits and hazards of a charge.
Skills: Crayghe recieve a +2 racial bonus on Listen checks, and a +8 racial bonus on Spot checks.

Looks good on an initial scan, but one question.

If the blindness is only permanent on a save failed by 5 or more, how long does it last otherwise? And what are the in-game penalties for being blind in one eye?

A timeframe would be until the damage is healed. As for the penalties, they are in the DMG (Page 27). That said, it will be added above.
