DMs: status quo or tailored?

DMs, do you run status quo campaigns or tailored campaigns (as described in the DMG)?

Care to summarize for those without the book in front of them?

I run a homebrew setting where I invoke house rules, add races, add classes, and subtract both, too. There are politics happening that cause governments to rise, fall, become corrupted, etc. whether the PCs are involved or not. I very much hope the PCs will paint a big enough stripe across the landscape that I'll have no choice but to record their name in the permanent logs of my world. There is also a very real chance this campaign will be the last run in my world because the arc will alloy for the utter destruction of the cosmology if the PCs don't do something about it.

I presume that is 'tailored', but it might be 'other'.
 


A mix. Some aspect are the way that they are because I planned the world that way in the absence of any consideration of the PCs. Some things are purely the result of thinking it would be a fun challenge well addressed to the PCs (frex, I thought a rival party with fundamental similarities to the PC party, but opposite genders, would be fun.)
 


I don't have the DMG in front of me, so here is a simplified example:

A dragon lives in the cave north of the city. The dragon is CR 15 regardless of the level or presence of the PCs. -- Status quo.

A dragon lives in the cave north of the city. The dragon's CR depends on the level of the PCs. -- Tailored.

I run a vast amount of status quo encounters, as I like to emphasize the setting in my game. This can create some unusual situations, but overall it's worked pretty well. I don't ever send the players on a quest that they can't handle, but a few characters have died after wandering off into a very dangerous desert alone (after being warned of the perils) and encountering some powerful monsters. However, there is a good mix of levels across the board, so epic level characters don't get a free ride.
 


There are lots of status quo aspects to the campaign, however, they arn't usually statted out as such. IMC it is well known that a group of black dragons inhabit the west swamps and rule over lots of lizard-folk, however, I have no details on it because I know the PCs arn't stupid enough to run off and attack them.

If I expect the PCs to do something, its tailored, however. I recently had a villiage ask for their aid in stopping a group of hobgoblins. For some reason the dungeon they went through was perfectly designed to challenge a group of their particular level and abilities. ;)
 

Mostly Status Quo, the world is set up and things are going on. If the players get involved and are smart they will find things they do that effect things. If they are not, well lets just say the Preists at the temple of life offer resurrection for good amount of gold. :D

Now if they players do get involved then might be some things that become tailored depending on what is going on. Sort like, if they make a major figure mad he's going to hire some thugs or another group to go after them. If that fails he might find another group a little tougher and tell them what the PC's use tactics wise and send them after them. Rinse and repeat as needed.
