I am now on campaign # 28 and only 3 didn't go beyond mid-level.

The odd one that does not usually is due to a premature end for one reason or another.

Not in a very long time. We did in our first OD&D campaign (actually got up to the Masters boxed set). And in our first AD&D (1e - 2e) campaign (peaked at about 14 - 17th level). But since then, we've only gotten as far as 7th level in various campaigns.

Heck, our current campaign with C&C is still at 3rd level.

Never had a campaign go beyond mid-level since I got past the munchkin god-killing days of my youth. Most of the groups I've been in have dissolved before we got further. The one really long running campaign I played in (1e, six years weekly, then four more less frequently) we only got to 13th level. The DM was very stingy with XP and treasure.

If Mid-level is 10th level, then rarely - maybe four campaigns I've ever been in in 25 years reached higher than 10th level, and even then only one went to 17th, with the others stopping at 11 or 12.

Currently, we're switching between an 11+ level Eberron Campaign (my campaign), and a Forgotten Realms campaign where everyone started at 17th level. Both are rather unusual for us (another DM's game). More usually, by the time the DM gets to 9th to 11th level, he's wearing out of ideas, and another DM is getting ideas for a new game. Then, he burns out and the other DM is ready again, usually with an idea for all-new characters. :) This is the first time I've taken characters that have played from 1st level, played them to 10th, and then picked them back up to go from 10th onward.

9th to 11th lvls are retirement for most of the campaigns i run. but this is the right time as they are the Lords, High Priests, and Wizards of the land by then.

mid level for me is lvls 4 to 7.

No, but it is happening more often. My numbers are a bit skewed as we had several games that started and petered off when the GM saw something shiny (and I admit, I've been guilty of this once or twice when I get excited about an idea, but either it doesn't look as good in action as I imagined, or I find I don't have the time to make it as good as I want). And there have been some games which have been so bad, we didn't continue.

But, now that I have one group that I am the sole GM for, and another where I GM 60-75% of the time, it's happening more. My on the fly ToEE game finished the temple and are preparing for the GDQ series, with the characters 11 and 12. The average level for my Eberron game is 11, and I've started Age of Worms with plans to play to the end. The Eberron game my friend runs has also hit 11.

I break D&D into low level (1-5) mid level (6-10) high level (11-15) and REALLY high level (16-20). I would never in a million years want to play/run epic, and i wouldn't want to run or play anything over 15th. Most campaigns i've run end by 10th, although when running Return to the Tomb of Horrors i think they all started around 13th. We never finished the fight against Acererak.
