Druids & Wildshaping?

Ok, I'm trying to get a more accurate handle on what stats a druid uses while wildshaped into another creature. I've looked at the article on polymorphing from the WotC site but I want to make sure. For the following stats, does the druid use normal druid stats (D) or the wildshaped stats of the creature (C)?

The only C determined traits you've left off, AFAIK, are the natural weapons that go under Attacks and any Special Attacks (not SQ). However, you do adjust anything modified by the new Str/Dex/Con. That includes current HP, init, saves, and skills. IME it's best to do up a crib sheet for the animal forms you'll use the most.
 

You gain the EX Special Attacks of the creature, the natural AC and any bonus feats such as a wolves track feat

Hit points, hit dice use the druid stats but are altered by any change in CON

You also get the racial skill bonus's of the animal if any and the druids current skills are altered by the new stats
