Since my campaigns have never gone far into epic levels, I've never used the "official" epic rules. I still can't believe they allowed such poorly thought out rules to become SRD. Consider a Wiz 20 / Ftr 20. You would expect this to be equal in power to a Ftr 20 / Wiz 20, right? Nope, by the "official" epic rules a Ftr 20 / Wiz 20 can crush a Wiz 20 / Ftr 20 into pulp any day of the week. BAB stops at level 20. You gain an Epic Attack Bonus that stacks with BAB from then on, but BAB never rises above +20/+15/+10/+5. Your 35th level character, by the book, has +28/+23/+18/+13 (BAB of +20/+15/+10/+5 and Epic AB of +8 applied to each).
Anyway, I - and every DM I personally know - use a continuation of the normal rules into epic levels. In the end the only difference it makes is number of attacks, how poor a save is (since - by the "official" rules - all classes gain Monk-perfect save progression for all saves upon gaining level 21), and a few other minor things. As has already been stated above, the number of attacks has never become an issue, having never reached more than six per round - and that only for a single PC (at least in my campaigns; for some DMs I know it has gone higher).
