It was in print. You can usually get it cheap on Ebay. It was done for 3.0 rules.

I've got the print version. Hefty hardcover, decent quality. It's one of my favourite sourcebooks.

I like the fatigue and infection rules. Makes trudging around with a heavy load all day actually have the effect it should, and in games without magic the infection rules make seeking out medical treatment a necessity after a wound.

Increased realism, which isn't everyone's cup of tea, but the extra rules are all pretty simple.

That's not the case. From Stone to Steel is one that I watch periodically because I recommend it often to people and like to steer them toward an available copy when I do. When it is available on ebay or through a used book online service, it often goes for around cover price (I've seen higher for near-mint condition copies).
