How I've been running Stealth

NMcCoy- If a rogue used stealth and melee attacked a creature on a dark night and the creature didn't have darkvision or low-light you'd allow that rogue to re-stealth right in front of the creature correct?
Yes, with a move action. They'd probably want to actually move when they stealth, though, as I would likely have the monster attack the square they were last seen in barring any better information. If the stealth check was successful, the rogue could do a full movement without provoking an OA as they'd be hidden right from the start of the move action.

So the rogue hides, sneak attacks someone, and remains hidden to the other 99 (unless his sneak attack forces him to leave cover - assume he's throwing a dagger from cover).
No - making an attack reveals you to everyone, not just the target.

Venturing into quasi-houserule territory based on the Perception skill description: As a standard action, I'd allow a Perception vs. passive Stealth check to fully spot a hidden character, rendering them non-hidden to you - effectively, taking an action to retry the opposed check with the perciever in the active role.

As I also pointed out, my biggest change would be to assume that hidden=/=invisible, unless you already have total concealment (which would only make even more difficult to have your location pin-pointed). So, hiding with concealment only gives CA, but you won't gain the benefits of being "invisible" (nor will you use the TWYCS rules). Your enemies can target you as normal, but with the usual -2 penalty for concealment.

That's close to how I am running it at the moment as well. I think that applying TWYCS rule only for characters hidden behind Total Concealment/Superior Cover is both consistent with the RAW and eliminates a lot of the problems with Stealth. Letting stealthed characters behind normal Concealment/Cover make attacks with CA is good enough not to nerf the rogues.

In terms of balance, I agree. On the WotC boards someone suggested making Stealth read 'If you have cover or concealment, you have combat advantage' and removing the rolling altogther! Rather nice I think :p

In terms of RAI, I've tweaked my streamlined rules and will be glad to receive opinions.
