How many rules mistakes do you make in an average game session?

Not counting minor mistakes like forgetting the +1 attack bonus from the cleric's bless spell for 1 round. Or miscalculating your carrying capacity by a couple pounds.

Just counting moderate to major errors; those annoying/aggravating mistakes that make you cringe, curse, or roll your eyes.

And separate from the clickable poll, how often do you make a really serious rules mistake? Something that can make or break the adventure, or makes a life or death difference for a character?

Ahhhh, just read the "just counting moderate to serious errors"... in that case, probably 1-2 errors. Anything else is minor.

I just had a big goof-up this past session. I told my players that a half-ogre was Medium sized, instead of the Large size w/ 10' reach that I should have had. Got called on it by my resident rules lawyer when he went and looked it up. Oh well, that's the way it goes at times, and in honesty, it worked to the players advantage, a 10' reach would have been MUCH worse.

I've been playing for just a year now, and I make plenty of small mistakes every game. I forget to add in bonuses from Haste and other spells, for example. But there are too many experienced players around the table for me to make major goofs very often.

However, one recent mistake that nearly cost the life of a party member got past everyone. My two-weapon wielding rogue/ranger had been possessed by a demon and attacked our sorcerer. For 54 points of damage. But I had made a full-attack instead of a surprise attack (standard action), which I later remembered was a big no-no.

I ended up last when initiative was finally rolled and that saved the poor guy's life. I still feel kinda bad about it.

I listed 11+, since I'm sure at least that many would occur based on our playstyle. We don't use a great many of the rules (no miniatures, and thus no rules relating to them), and we treat the rules as more of a friendly suggestion than cannon. Whatever makes the most cinematic impact is usally allowed, and DCs and the like are more or less eyeballed for dramatic effect.

A few as the campaign starts but I really try not to make the same mistakes over and over so the number of mistakes uisually goes down as the campaign goes on.

Roughly one per session, whether playing or DM'ing. We mostly only discover it after the fact, but if I ever have a major lack of clerity on a rule, I'll stop us long enough to look it up, or if it takes more than a couple of minutes, I'll make as fair a spot ruling as I can and continue and look up the answer later.

You mean like forgetting to roll a massive damage check after an 89 point Fire Seed on a Frost Giant, or not including Strength Bonuses when rolling critical hits? That kind of thing?
