How to roll every type of die in an attack?

Fighter/Rogue with Magic Initiate (Bless, Toll the Dead)

Attack (d20 & d4)
Longsword + sneak attack (d8 & d6) <--- Oops can't sneak attack with longsword. Hmm.
2nd Attack: use both hands (d10)
Action Surge
Toll the Dead (d12)

Rapier does it but the d12 for toll the dead is a second attack and Extra attack is not a d10 unless you drop the rapier and draw along sword. Not supper functional.

On, lets try this then
Battlemaster fighter 10/war cleric 8

d20 attack roll
d12 great axe
d10 superiority die (lvl 10-17 battlemaster)
d8 divine strike (lvl 8-13 war cleric)
d6 combat inspiration ( lvl 3-4 valor bard ally)
d4 divine favor (from war domain spells, requires concentration)

If we need to avoid « cheating » with an ally casting enlarge or giving a combat inspiration die, or gaining an extra d6 from a frost brand, Etc., there are a few way to achieve it with xanathar guide and sword coast for things like absorb element, monster slayer ranger, booming blade but otherwise, I can’t see how to do it without exterior help with PHB only.

I feel bad for the lvl 4 Valor Bard partying with a level 18 Fighter/War Cleric but I am going to Imagine your friend multi-classed a whole campaign just so you could pull this off...
 

Oath of Vengeance Paladin 3 / Valor Bard 10
d20 attack roll
d12 great axe
d10 Bardic inspiration
d8 Smite
d6 Initiate Hunter's Mark (Oath of Vengeance, your concentration)
d4 divine favor/Bless/enlarge from an ally of any level with one of these spells making it a lot easier to pull off.

Now its repeatable and almost all one character but the d4 which is easy to get, however as a strength paladin bard with a great axe, don't expect anyone to like you. lol

Of course you could do:

Battle Master Fighter 10 / Pact of the Blade Warlock 3 with Eldritch Smite invocation
d20 attack roll
d12 great axe
d10 Superiority dice
d8 Eldritch Smite
d6 Hex
d4 divine favor/Bless/enlarge from an ally of any level with one of these spells making it a lot easier to pull off.

Still scratching my head on how to have a d8 rolled on a skill check. Otherwise we could have something like:

d20: ability check
d12: bardic inspiration (bard lvl 15+)
d10: Dark One Own Luck (Fiend warlock 6th level feature)
d8: ???
d6: Hero Point (optional rule from DMG)
d4: guidance
 

This thread has me wishing for a spell or ability somewhere that involves rolling damage of 1d4 + 1d6 + 1d8 + 1d12 + 1d20. (Everybody knows the the d10 isn't a real die.)
 

3rd edition miss chance as a 5e house rule.

If you're going to try to get both dice of a percent die in (not the original challenge, but a reasonable variant) then house rules like that are the only way to do it. If you do roll percentile dice with the attack, then you can get an 'all combat long' roll of all dice. D20 base, D12 weapon, D100 for miss chance, D8 smite, d6 hunter's mark/hex, d4 bless.
