The issue of routineness is open to debate I suppose, but personally I think that if you can do it every time you encounter a gigantic monster, that seem a little too routine for my tastes. YMMV.

This. I'm honestly considering the w/in one size category restriction, which IIRC is what 3e trip rules used as well. In the case of an effect that targets will, I might be more willing to interpret broadly.

The thing about SotC is that you have to climb all over a monster and deliver specific attacks to specific locations before you can hurt it. It plays out more like Legolas taking out an Oliphant by himself in Return of the King than it does pushing a Tarrasque over with your shield. Precision, repeated strikes and creativity.

I'd want to represent something like that with a combination of combat and skill challenges, making athletics and acrobatics checks to reach the vulnerable spots on a monster, and maybe nature or arcana checks to know where they are. This would be coupled with standard attack and damage rolls... you know, the more I think about it, the more I like that idea.

I was thinking along the lines of an immediate saving throw to resist, which as close as you get to an opposed check in 4e.

Okay, see that sounds all kinds of awesome and I will completely steal it. It's about a million miles away from Jeremy's apparent ideas that "If it's not at all, then it should be able to anything to anything, no matter how little sense it makes!". Me, I'll sacrifice some "tight tactics" for a bit of versimilitude, thanks. My players, even more so.

Jeremy - Yeah but do you fall on the freakin' floor? Or drop to your knees? I sure don't. Do you stagger away from the cutting-board because there's blood on the onions and you're going to need a bandaid on your finger? It just seems extremely ludicrous. Particularly with four-legged creatures.

Case-by-case I say. If it makes sense, it makes sense, if it doesn't, it doesn't.
 

I just think that 4e is a tad more dynamic than previous editions.

Knocking something prone doesn't need knocking it on its tush; maybe the halfling got it all tangled up in its own legs (yeah... the CUBE....)

Sliding something doesn't require a phsyical act, maybe its the attacker faking,bluffing, or out-maneuvering the opponent... Its just simpler to say...Slide the target 2 squares...
