Looking for TSR info

2. I've seen Ward mentioned in the context of 'TSR President,' or something very like it. Was this his actual title during that time? If not, what was? Either way, who was the lead decision maker at TSR? Was it Ward, Lorraine Williams, or someone else entirely?

The only positions I know of that Mr. Ward held at TSR were:

Designer (TSR, Freelance, SSI), Purchasing, VP Creative Services, Marketing, Manager, Novelist

And just as a follow up. During her "reign" Lorraine Williams held the titles of:

President and CEO of TSR Inc.

Since I started playing D&D 2 months before 3rd edition came out I can't tell you much about the products he made a tsr. But theirs links on that page to the descriptions.
