Monk Attack and Damage

I've read somewhere, can't remember where, that monks get 1.5 x STR on every unarmed attack and all attacks are considered being made with their main hand (not off hand). I've looked and can't seem to find any reference to this.
Can someone point out where this located, page number, book, etc.

There is a feat which gives 1.5x dam with unarmed attacks but the monk can't use flurry of blows at the same time. I believe it was published in Dragon Magazine but I don't know which one off-hand. (get it? off hand :) )

PH page 8 details how strength is applied. The monk's unarmed attack does not qualify for the x1.5 strength bonus.

PH page 39 says there is no such thing as an off-hand attack for a monk attacking unarmed (top of second column). It also says under the Flurry of Blows section (middle of second column) that the damage bonus for the off-hand is not reduced when the monk makes a flurry of blows (it is reduced if the monk uses any non-monk weapon in either hand).

So a first level monk with an 18 strength gets either one unarmed attack with a +4 attack bonus and a +4 damage bonus (not +6), or a flurry of blows with two attacks at a +2 attack bonus for each (all attacks get a -2 penalty when using flurry of blows) and a +4 damage bonus for each (not +4 and +2).
