Ezabard cannot use shield as he used his reaction to attempt to counterspell Vecna earlier and has no reaction remaining.

As for Vyrlim, you are correct, @sleepyInsomniac did say he was using his reaction to impose disadvantage, so I'll update the turn.

Unfortunately, it won't help Ezabard in the long run by that alone... The green shadow kills him by reducing his strength to 0, regardless.

This really falls on Pyre and her gaining sneak attack damage--somehow...
From sneak attack description: "You don’t need advantage on the attack roll if another enemy of the target is within 5 feet of it, that enemy isn’t incapacitated, and you don’t have disadvantage on the attack roll." He should get sneak attack damage"

All shadows are within of 5 feet of an enemy.

The paladin can cast revivify (but let's coordinate so it's not counterspelled)

Not where Pyre was located when the shadow first appeared. Here, this will help:

The Green Shadow was summoned within 5 feet of Pyre. Moved (provoking OA from Pyre) to spaces 1,2,3,4,5 and finally into its final position between Selossen and Vyrlim, at which point it attack and killed (for now) Ezabard.

Could Pyre have thrown a dagger and add sharpshooter damage instead?
Good thought, but no. Opportunity attacks are melee weapon attacks unless you have a feature that allows otherwise (like War Caster).

EDIT: while technically the dagger is a melee weapon attack, the attack must occur before the creature leaves your reach. Since Pyre would be making a ranged (thrown) attack against a creature within 5 feet of her, she would have disadvantage on the attack.

If she accepts the -5 to attack and disadvantage on the roll... maybe?

Actually, no. Her first roll (shown in the turn above) was a 3. With the -5 penalty, that 3+11-5=9 misses AC 12.

Also, sharpshooter cannot be used with thrown weapons, only ranged weapons.

I have war caster but it didn't provoke for me.
