When I worked for Sun, I was stunned they didn't track sick days. They trusted you not to abuse it. Even if it was a mental health day.
Never heard of anyone abusing it.
My Dad used an honor system for leave & sick days in his business. And he tried a time clock for a month before turning it into a paperweight.

His employee turnover was extremely low.
 

Apparently when you offer unlimited vacation days, the actual number of vacation days taken goes down.
"Unlimited vacation days" is often a scam, as they add the codicil that the leave must be authorized by a superior, and must not impact productivity. In practice such authorization is rarely provided as productivity is, of course, always impacted by an emplyee absence.
 

I’ve seen the unlimited vacation days work out well at some places but I think the big thing is that you can never really be sure it’s going to be approved. I’ve seen folks take sabbaticals but I also know they did a lot of work to make sure they had coverage in their jobs, and they had to plan it in advance, etc. But on the average, I would say there are more people taking a greater than average number of days off, and it’s nicer than having to do a hard count of days available for vacation or sick time.

A lot of places are poorly run. They can't handle the most minimal disruption.
 


My wife is notorious for me having to poke her to get her to use some of her vacation days. We take a couple weeks off a year (once around our anniversery and once near Halloween) and she still manages to accrue more than she uses.
Since I moved to working from home 80-90% of the time, i definitely struggle to use my vacation time. I also been with my company 23 years now and so I get a lot of it.

Same. It's 27 years here. I'm currently at 25 vacation days a year, can carry 50 before cut-off, and generally get an extra week (sometimes 2) between Christmas and New Year as the uni is shut down.

A little bit of that, and a lot of bits of compulsive overworkers we sometimes had to chase out of the office so they would sleep. Particularly important if you are a big believer - as I am - in the diminishing returns of time expended for knowledge workers ( IME, it eventually goes negative )...
