Reaper's Strike and Ongoing damage, how does it work?

Ongoing Damage:
Some powers deal extra damage on consecutive turns after the initial attack.

So, to the OP, its option B.

Since the Fighter's power damage the creature on the creature's turn, they would not take the damage until their next turn. The creature would get a chance to save at the end of their turn however.

If the creature did not make their save on the first turn and they were still adjacent to the Fighter on the next turn, then the Fighter would deal the 1W and the 10 ongoing.

No, but it doesn't seem like it should be that way.
Start of turn involves a bunch of stuff that must happen, and everything in 4e is sequential ..
IMO, the start of turn should be automated, i.e. no decisionmaking.

So it isn't that way, and there are some effects that can be ordered for maximum advantage, but there's got to be a standard order that's optimised, and the effects resolved accordingly.

Hmm .. even with such things as "regen 5 while bloodied" and "ongoing 10", you'd usually take the damage first unless you're between 6 and 10 hp (so you can remain standing) or within 10 hp of -bloodied (so you die if you don't regen first).

Remember, Regeneration doesn't do anything if you're at 0 or lower. So if your hit points are between 1 and 10, you want to Regenerate first, and if you're at 0 or lower, it doesn't matter, 'cos you aren't Regenerating anyway.

But in the case of Reaper's Stance, there is no "initial attack"; the creature gains the ongoing damage by starting its turn in the square adjacent to the Fighter.

In this case I would say that when the initial damage takes place would be considered the initial attack. However, I can agree that it is not completely clear cut. It seems like the best interpretation to me though.

Ah, yes ... that makes it easier .. give regen a high priority unless hp is almost full.
Soon as damage bloodies you, the regen kicks in.

edit:
Crap .. that doesn't solve the consecrated ground problem.
