The fundamental problem is that Rust Monsters are flat-out dumb creatures IMNSHO. You will never maintain sanity if you allow such things in your universe.
If you are running a vanilla Greyhawk campaign, I am afraid the odds that that a seasoned dungeon delver has met a Rust Monster is pretty darn high.
